FSL End of Semester Report

Please complete the form below based on this past semester only (previous semester should NOT be included).

Per the FSL Handbook "Each semester, the chapter president is responsible for completing the end of semester report. Chapters will report philanthropy dollars, community service hours, programs held, membership information, and more. The report will open at least one week prior to study day and will be due one week after the last day of finals. Failure to submit the end of semester report will result in a $100 fine."
