Applicants must submit a grade sheet for current Spring 2021 grades. Download the grade sheet form. Take the form to all current professors to fill in your grade and sign. If you are in an online only class the professor can electronically sign the form in Acrobat and send it back to you. Once all signatures are collected, scan or take a photo of the form.
