  Step 3—Complete and submit any other required documentation, such as the Assent and Consent Forms (below), copies of surveys, etc. You will need to upload these within your Slate application.

  • Children (under 18 years of age)
  • Child Oral Assent Form
  • Children Written Assent Form
  • Parent Consent Form
  • Adult Informed Consent Form
  • Research Dissemination

• Step 4—Complete the Slate Application, upload all relevant documents, and click Submit. You will receive an email confirming your submission.
