First-Time Forms: 

To start your benefits, complete the Microsoft Form for the benefit you would like to utilize. You will only need to complete this form the first semester you plan to use the benefit. Check the section below for forms that need to be completed each semester.

No reoccurring forms are required for the Alabama GI Dependent Scholarship or Military Heroes Scholarship apart from the yearly FASFA.
