DSS FORMS & PROCESSES

Below are DSS Forms and Procedures to reference and utilize as needed for services.

Accommodation Request Process: Explains and provides information for the accommodation request process and student responsibilities each semester to meet with DSS Staff to complete notification to faculty members. 

Accommodation Review Form: The form is utilized by students who are currently receiving accommodations through the DSS Office AND are requesting an additional accommodation to be reviewed. 

Alternative Media Request Process:disability-support/alternative-media-request-process.docx Explains and provides information for students who are approved accommodations related to alternative media formatting.  Includes directions and a request form.

Consideration Process: Relating to students who are experiencing a temporary condition which limits there abilities, the form explains the process for students who are requesting considerations. 

Consideration Review Form:  The form is utilized by students who are currently receiving considerations through the DSS Office AND are requesting an additional or extensions of the consideration.  

Disability Related Absence Accommodations: The form provides information to students who are approved accommodations related to flexibility with attendance policies, including student responsibilities, communication, and documentation. 

Housing Accommodation Process: Explains and provides information for the housing accommodation request process and student responsibilities once approved for accommodation. 

Impact Statement: Form to be completed by students who are requesting accommodations.  Students are to upload a completed form in the DSS Portal. 

Meal Plan Accommodation Process: Explains and provides information for the meal plan accommodation request process and student responsibilities once approved for accommodation. 

Self-Advocacy & Communication to Faculty of Your Accommodations:  Provides a suggested process for how to self-advocate with your professor to explain and discuss your accommodations for the class. 

Student Reasonability for Testing: The form provides information to students who are provided testing accommodations.  It includes instructions on how to request to take a test in the DSS Office and what items are provided and restricted within the testing area.
