Please be advised that the month of February 2020 will serve as the University's annual open enrollment period for adding, dropping, or making changes to your supplemental insurance coverage.  All changes will be effective March 1, 2020.  The following coverages are included during this open enrollment period: AFLAC - accident and hospital; Air Evac; Allstate - cancer/intensive care; Dearborn National - short term disability; VSP – supplemental vision; Guardian – voluntary life insurance.

The renewal date for the University’s group voluntary life insurance coverage with Guardian is March 1, 2020.  There is nothing that you need to do if you desire to maintain your current coverage.  For informational purposes: any employee who moved into a new age bracket will experience an increase in premium.  To review a rate chart, please see below.  In addition, if you currently have voluntary life insurance coverage on your spouse, the premium for your spouse's coverage will also increase based on your new age bracket.

A representative from The Hunter Benefits Group will be available to talk with anyone interested in adding and/or changing his/her supplemental benefit options during the following meeting periods in the University Events Conference Room in the GUC:

Thursday, January 30, 2020: 9AM - 11AM and 2PM - 4PM

Payroll Deduction Information:  It is important to note that the University deducts insurance premiums a month in advance.  Therefore, enrollees may submit an application at any time during the month of February.  However, in order to avoid paying back-premiums, the following dates are to be noted. Biweekly paid employee - completed applications and/or changes should be submitted to this office by 02/04/2020. Monthly paid employee - completed applications and/or changes should be submitted to this office by 02/20/2020.

Air Evac Membership - Open Enrollment 2020: The University offers a discounted group rate for eligible employees to secure a policy (membership) with the Air Evac Lifeteam, which provides emergency medical transport via air for serious illnesses and injuries.  During the month of February 2020, any employee not currently enrolled will have the opportunity to purchase a membership for the first time.

The price for a membership (or renewal) as part of the UNA group is an annual, lump-sum payroll deduction of $50.00 which will be payroll-deducted on March 6, 2020 for bi-weekly paid employees and March 1, 2020 for monthly paid employees. If you are interested in purchasing a membership, simply print and complete the membership application for a one-time annual payroll deduction and forward to the Office Human Resources at the address below no later than February 20, 2020.  Any membership purchased last year will be automatically renewed unless revoked by the employee/policy-holder through Human Resources.

Air Evac Lifeteam is the greater Shoals area’s provider of air ambulance service.  Air Evac provides emergency medical transport to an appropriate medical facility based on the nature of the injury/illness.  The air medical services provided by the University’s Blue Cross/Blue Shield health insurance cover ONLY transportation services for an already hospitalized patient to a hospital near a his/her home should he/she be hospitalized while more than 150 miles from home.  In contrast, Air Evac services are used to transport an injured/ill patient to the most appropriate treating facility.  For this type of transport, Blue Cross/Blue Shield insurance will only cover up to 80% of the cost of Air Evac’s services minus the deductible, typically leaving a balance of at least $3,000.  The purchase of an Air Evac Lifeteam membership ensures that a covered member incurs no out-of-pocket expenses for these services.  In addition, an Air Evac membership covers an entire household provided all household members are listed on the enrollment form.

Specific terms and conditions exist for this type of service.  Please make sure that you read these terms and conditions on the membership application linked below completely before making a decision to purchase the membership.  If purchased, your membership will be valid for the period April 1, 2020 through March 31, 2021.  Thereafter, your membership will automatically renew unless you direct the Office of Human Resources to cancel it.

If you have questions, please contact Becky Price at rjprice@una.edu or extension 4650.
