Academic Status Appeal Form

Any student who has been suspended for one calendar year or any student who has been suspended for three calendar years and has completed one year of the suspension period has the right to file an appeal of that status and to request that the Readmissions Committee hear the case. The Readmissions Committee, upon hearing the case, has the authority to permit readmission of an academically suspended student before the expiration of the period of suspension. However, the Readmissions Committee exercises that authority only when the appealing student can present substantial evidence of unusual mitigating or extenuating circumstances which led to the student's academic difficulties or when the appealing student can present substantial evidence of changed circumstances supporting the likelihood of the student's future academic success. ONLY ELIGIBLE APPEALS WILL BE HEARD. COMPLETING THIS FORM DOES NOT AUTOMATICALLY WARRANT A HEARING!  

 

This form must be submitted by 4:30 p.m. on Friday, August 10, 2018. No late appeals will be accepted.

In order for the Readmissions Committee to evaluate unusual or extenuating medically related circumstances (personal or family) that led to your suspension, you must present full documentation of all medical evidence to this Committee.
If medical evidence needs to be provided, you may drop that by the Office of Enrollment Management in Commons 313. It will be included with your documentation for the committee.
