Suspension Status Appeal Form

Any student who has been suspended for one calendar year or any student who has been suspended for at least three calendar years and has completed one year of the suspension period has the right to file an appeal of that suspension and to request that the Readmissions Committee hear their case. The Readmissions Committee, upon hearing the case, has the authority to permit readmission of an academically suspended student before the expiration of the suspension period.

However, the Readmissions Committee exercises that authority only when the appealing student can present substantial evidence of unusual mitigating or extenuating circumstances which led to the student's academic difficulties or when the appealing student can present substantial evidence of changed circumstances supporting the likelihood of the student's future academic success. ONLY ELIGIBLE APPEALS WILL BE HEARD. COMPLETING THIS FORM DOES NOT AUTOMATICALLY WARRANT A HEARING!

This form must be submitted no later than 5 business days prior to the start of the term. Late appeals will not be accepted.

In order for the Readmissions Committee to evaluate unusual or extenuating medically related circumstances (personal or family) that led to your suspension, you must present full documentation of all medical evidence to this Committee.
If medical evidence needs to be provided, you may drop that by the Office of Enrollment Management in Commons 313. It will be included with your documentation for the committee.

Please make sure all required fields have been filled out correctly.

Thank you for your submission.
